As a client, you have the right to access information about your care, request copies of your records, and submit concerns regarding professional services. The resources below are provided to help you understand and exercise those rights.
Requesting Records
You have the right to request copies of your counseling records, subject to applicable state and federal laws. To request records, please submit your request by email to: kate@autumnridgelpc.com
Requests will be reviewed and processed in accordance with applicable privacy regulations and professional standards. Additional identity verification may be required before records can be released.
Office of the Attorney General
Consumers may also seek information regarding consumer protection rights through the Texas Office of the Attorney General.
The No Surprises Act
The Texas Behavioral Health Executive Council (BHEC) investigates and
prosecutes complaints against licensed professional counselors in Texas.
Complaints may be submitted to the Council using the official complaint
form available on the BHEC website.
