As a client, you have the right to access information about your care, request copies of your records, and submit concerns regarding professional services. The resources below are provided to help you understand and exercise those rights.

Requesting Records

You have the right to request copies of your counseling records, subject to applicable state and federal laws. To request records, please submit your request by email to: kate@autumnridgelpc.com

Requests will be reviewed and processed in accordance with applicable privacy regulations and professional standards. Additional identity verification may be required before records can be released.

The No Surprises Act

The No Surprises Act of 2023 entitles all clients to a financial estimate of services. Visit www.cms.gov/medical-bill-rights/know-your-rights/no-insurance for more information

Consumer Protection Resources

Texas Behavioral Health Executive Council

The Texas Behavioral Health Executive Council (BHEC) investigates and prosecutes complaints against licensed professional counselors in Texas. Complaints may be submitted to the Council using the official complaint form available on the BHEC website.
